Record Requests

Some records are maintained at Sycamore Fire Station #1, located at 535 DeKalb Avenue. Please call 815-895-4514 during normal business hours to inquire if records are available. Verification of identification and/or official documentation may be requested prior to releasing records.

Fire & Incident Reports

Property owners, managers, and tenants may request fire reports from the Fire Department. Others may request fire reports through the Freedom of Information Act (FOIA) process. For more information about FOIA, please call the City Clerk at 815-895-4515.

Ambulance Patient Reports

Due to HIPAA and Sycamore Fire Department Privacy Practices, only patients themselves, patients' parents or legal guardians, or patients' authorized personal representative may request patients' medical care reports by completing an Authorization to Release Protected Health Information form and submitting it to the Fire Department, along with official proof of identity and/or other required documentation.

Ambulance Billing

When an ambulance responds to an emergency medical service request and provides patient treatment and/or transportation to the hospital, the City of Sycamore assesses a fee. EMS Management and Consultants (EMS|MC) provides ambulance billing services on behalf of the City.

• Patients themselves, patients' parents or legal guardians, or patients' authorized personal representative may call EMS|MC at 866-827-8469 for ambulance billing-related matters. 
• Law firms, insurance companies, record retrieval agencies, and other legally authorized entities, seeking ambulance billing-related information should a request online through Chart Swap (www.chartswap.com).
